Professional Development Application
At the bottom of
this page there is a link to the pd application form.
You may download
the application by right clicking and selecting 'save Target as...' or you may
open the application in Internet Explorer by simply clicking on the
link.
Fill in the information in all of the appropriate grey
squares and print out the application.
Sign the application and have your immediate supervisor approve and sign it.
Make sure that the application reaches the ATA office
by the deadline as outlined below.
NOTE TO
APPLICANTS
1. Teachers wishing to apply for assistance should
read the document "Guidelines for
Administration of
Professional Development Fund".
2. Applications should be delivered to the ATA
Office prior to 4:00 p.m. on the appropriate deadline
date.
3. Applications MUST be received by the deadline for
each period:

NOTE: If
the deadline falls on a weekend or holiday, you must submit applications by the
last school day
before
the deadline. Applications must be at the ATA Office at 4:00 p.m. on the date of
deadline.
4. This fund will not normally be used to support
individuals to be absent from their duties for more than five
days
in any
school year.
5. Maximum annual benefit will not normally exceed
$1,200.
6. Normally, approval will be granted for only one
major activity within any three (3) year period.
7. The maximum amount granted to a teacher during
any 3 year period -- ie. the current calendar year plus
the
previous
two calendar years, shall be $2,100.
8. NO advances will be made.
9. Under normal circumstances, expenses must be
claimed within 30 days. IF WARRANTS ARE NOT RECEIVED BY THAT
TIME, THEY WILL NOT BE REIMBURSED UNLESS AN
ACCEPTABLE LETTER OF EXPLANATION IS RECEIVED.
10. If, for some reason, your approved activity is
cancelled, please send written notification to the ATA
Office
immediately, so that others may make use of the
Fund.
11. Major trips shall be defined as those involving
more than three consecutive school operating days and/or
involving an expenditure of more than $700.
RATES ALLOWED
A. Transportation: as indicated on the
form
OR 18cents/ km, for all other points (as calculated
on road maps) OR economy air fare outside 550 km.
WHICHEVER IS LESS.
B. Meals: $35.00 / day for each conference day
indicated in the literature.
C. Accommodation: $80.00 / night. (Normally equal to
the number of conference days provided conference location exceeds 200
km
from
Lethbridge).
D. Registration: Total fee paid including banquets,
etc., but NOT including membership fees or costs to non-members (receipt
or
cancelled cheque required)
